Have facility where death occurred or hospice nurse, if death occured at home, call 1-800-300-1655 or 972-241-9100 to request our care team take your loved one into our personal care.


If your loved one died in the DFW Metroplex or surrounding North Central Counties and you do not live in the area but need to make arrangements, call 1-800-300-1655 or 972-241-9100 to speak to the Veterans Funeral Provider Specialist.


Bring to the appointment your loved one's social security card, driver's license and separation papers (DD-214) from the military.

  • Fax DD-214 to V.A. National Scheduling Office To Arrange Burial
  • Grave space or Cremation Niche at DFW National Cemetery
  • Outer Burial Container at DFW National Cemetery
  • Opening and closing of the grave or niche at DFW National Cemetery
  • Grave marker or engraving of niche front at DFW National Cemetery
  • Use of shelter for 30 minute service at DFW National Cemetery
  • Military Honors (folding of flag, presentation of flag and recorded playing of taps)
  • Assisting the family with V.A. burial benefits forms
